Nearly 5,000 bottles of popular iron multivitamins have just been recalled due to defective packaging that could lead to “risk of serious injury or death from child poisoning.”
The U.S. Consumer Product Safety Commission recalled 4,700 bottles of Bariatric Fusion Iron Multivitamin on Thursday, Sept. 11.
Why the bottles were recalled
The multivitamins, manufactured by VitaQuest International, must be in child-resistant packaging, as required by the Poison Prevention Packaging Act.
The agency has concluded that the affected bottles violate “the federal standard for child-resistant packaging because the bottle caps are not child-resistant, posing a risk of deadly poisoning” if young children swallow the contents.

The recalled bottles
The USCPSC has recalled 4,700 bottles of Bariatric Fusion High ADEK Multivitamin.
The recalled bottles include the lot numbers 0066J4, 0065J4, 0453B5, or 0370B5. The numbers are printed at the bottom of the bottle.
There are two types of Bariatric Fusion dietary supplement bottles: high ADEK multivitamin capsules (90 and 270-count bottles) and One Per Day bariatric multivitamin capsules (90-count bottle), both with 45mg of iron.
What’s more, the only bottles affected come with a smooth cap that lack the “push down & turn) embossed lettering.

Fortunately, the USCPSC says in the recall notice that no injuries or incidents have been reported due to the defective bottle caps.
The bottles were purchased on Amazon.com, bariatricfusion.com, and other websites between January 2025 and June 2025. The prices per bottle range between $23 and $57.
The bottles are manufactured by New Jersey-based VitaQuest International, LLC, and distributed by Connecticut-based Blueroot Health, Inc., in Middletown, Connecticut.
